Aim and Purpose
The aim of the discharge planning process examined in this nursing assignment is to decrease the staying time in the hospital including unplanned readmission into the hospital and to develop the coordination of assistance of discharge from the hospital through connecting the passage within the hospital and place of discharge. The perfect discharge planning procedure continues to involve patients and their family members during the change from hospital to home, with the purpose of overcoming conflicting issues including preventable readmissions (Negara et al. 2019). Discharge planning encompasses recognizing any services, supplies, and follow-up that might be required for the proper transition of the patient home or elsewhere.
Role of Registered Nurse
Taking extra care of patients has been considered as the prime duty of the nurse. Discharge planning is crucial for making the patient for leaving the hospital and for supporting healthcare providers concerning follow-ups including additional treatment. According to Thoma & Waite, (2018) proper discharge procedures usually assist to circumvent the requirement for re-hospitalization of the patient during the coming future. Registered Nurse usually evaluates and recognize the needs of the patient, and later registered nurses generally implement and monitor the medical plan of the patient including their treatment. Understanding the Procedures in discharging planning process
Registered nurses generally do not work directly with the patient throughout the time of staying at the hospital. Therefore, registered nurses used to work closely with the doctors for ensuring the plan of discharge and everyone who are usually associated with the patient should know about the procedures (Brummelaar et al. 2018). They also address these plans with the patient’s family members and also inform any information regarding the further healing of the patient. The registered nurse generally practiced in respect of the NMBA and NSQHS standard codes and guidelines for providing better service.
Arranging patient’s referrals
During the time of initial treatment in a hospital or other health care facilities, a patient might need more treatment from a different provider. A registered nurse who usually handled discharge planning assisted to manage patient referrals for further treatment from the medical services which generally incorporated with home care, physical therapy, primary care, and many more.
Responsibility for teaching family
The registered nurse always has a responsibility in every stage of treating patients and during the time of treatment a patient often needs further treatment and also care following the discharge. In those scenarios family members often come forward for help but it is the responsibility of registered nurses who maintained the discharge planning by providing proper knowledge to the family maintaining the NMBA and NSQHS standards. This knowledge is generally related to caring for the patient like wrapping the wound properly, checking pressure, and reading glucometer.
Teamwork
A registered nurse is not alone inside the hospital; she needs to work jointly with the team members in sequence to guarantee the best possible care. While planning for discharge, the registered nurse might hand over specific tasks to the patient’s authorized nurse, particularly if there are numerous discharges occurring at the same time. The registered nurse needs to work in a team and in order to maintain the discharge planning process along with the caring and following up treatments with the physician of the respected patients.
Discharge instruction for patient and family
The process of discharge planning involves early identification along with evaluation of patients needing support with preparing for discharge; cooperating with the family, patient, including healthcare team to promote discharge; suggesting alternatives for the ongoing care of the patient and also referring to other essential accommodations or services that satisfy the preferences and needs of the patient. The patient and family should be presented with a 24x7 emergency phone number, the patient should inform about the provider who will be responsible for the care after the discharge. As stated by Ragavan et al. (2017), during the time of discharge, both patient and family should be informed about the proper guidance regarding basic caring such as wrapping of the wound, checking pressure at regular intervals, reading glucometer, providing prescribed medicines as per the given instruction from the hospital, and also maintaining regular checkups after the process of discharge. It is also important to inform the patient family by making them understand the diagnosis and also ensuring the patient and family have an active care plan.
