Question

Task: Reflect in your nursing assignment on how organisational structure and culture can affect person-centred care delivery safety. Reflect on the skills related to health information systems. Develop awareness of the transition from a Diploma nurse to Bachelor's degree nurse through critical self-reflection.

Answer

Introduction
The term "safety culture" can be referred to as a subset of an entire organizational culture. It has been identified that organizational culture and its structure can significantly affect certain aspects of “person-centered care delivery safety” concerns. In respect to this concern, this nursing assignmentaims to address how an organizational structure and its culture can eventually affect person-centered care delivery safety contexts. Even, this nursing assignmentwill discuss the indispensable attributes related to HIS or Health Information Systems. Precisely, this entire nursing assignmentwill reflect upon my entire clinical experience and the knowledge that I have acquired throughout with an emphasis on Gibbs' reflective cycle (1988) to evaluate critical self-reflection. Alternatively, it can be claimed that the critical evaluation of this nursing assignmentis inclined to develop proper awareness of shifting from a concerned Diploma nurse to a responsible Bachelor's degree nurse over the period.

Part 1: Organizational structure and culturediscussed in the nursing assignment
Description

This is to be included in this nursing assignmentthat safety concern is often influenced by the organizational structure and its culture. At the beginning of my clinical placement as a concerned student nurse, I experienced a severe safety concern. Though I was not initially feeling comfortable discussing it, I think it is essential to promote a safety culture as we have gone through a really bad experience during those days. The institution where I had been assigned a clinical placement as a student nurse had experienced infections as a significant safety issue due to improper hygiene maintenance of its organizational structure, especially in the washrooms(Wang et al. 2020.p.140). Initially, neither I nor any other nurses were able to speak up about safety concerns due to utter hesitation. After 2 days, we altogether complained about the same safety issue to the organizational authority so that necessary action could be taken with immediate effect.

Analysis
In addition to this nursing assignment, it is significant to mention that the other nurses and I had taken 1 or 2 days to get accustomed to the organizational structure where we initially were placed as student nurses. After working there as a student nurse for two consecutive days, I felt that it was extremely essential to speak about it because the same would surely be considered a significant safety concern. Meanwhile, I had also discussed with the other student nurses who were also placed in the organization and all of us agreed to talk and convey to the concerned organizational authority about the complexities that we were experiencing due to experiencing significant safety concerns, i.e. infections. While working in the organization as a student nurse, the other nurses and I explored that the organizational authority and the concerned hierarchy of the organizational structure are quite cordial and they were open to receiving feedback. Based on my nursing assignmentlearning, I have come to know that the organization had introduced the Speaking up for safety culture into its eco-system. Precisely, it is essential to admit that such kinds of cooperative, cordial organizational culture had made it possible to speak up about safety concern to resolve it. According to my opinion in the nursing assignment, there are many kinds of interactive organizational culture that can be practiced in an organizational structure and the same enables the student nurses to reveal if they experience any types of safety concerns. As per my learning, ‘Speaking up for safety and Health’s Safety Culture Program are two significant organizational culture that enables to speak about safety concerns as also identified in this context (Jones et al. 2021.p.380).

Reflection
Based on the learning that I had consistently acquired throughout pursuing this course, nursing assignment and my entire clinical experience enabled me to feel confident while speaking up about any safety concern that needs to be resolved with an immediate effect. Precisely, I have felt the overall essentiality of resolving a safety issue as one an integral part of ensuring the operational effectiveness of an organizational structure and its culture. Due to this, I thought of revealing the crisis about the safety culture of the organization where I initially got placed to serve my responsibility as a student nurse. In a nutshell, I find it one of my responsibilities to unveil the significant safety concerns thatI experienced there. This initiative can also be referred to as a significant strategic way to promote a safety culture in an organizational structure.

Part 2: Health Informatics
Description

In this relevance, it is significant to mention in the nursing assignmentthat I have acquired a wide range of knowledge about Health Information Systems (HIS) while pursuing the course in which I have enrolled myself (Rantz et al. 2018.p.545). Initially, I got to know that a registered nurse is required to be an expert in initiating effective communication, and the ability to work in a team environment. The individual (i.e. a nurse) must be attentive to patient details, case history, etc. (Russ & Saleem, 2018.p.122). Even, a nurse must be proficient in technical skills with proper certification. Still, when I was introduced to the wide application of health information systems during my clinical placement, I got confused and was unable to figure out the patient flow system, patient browser, and Electronic Medical Record [EMR] in the initial phase.

Analysis in details
In this nursing assignment, I have already admitted that I, initially was hardly proficient in the handling of health informatics systems in a real-world nursing environment and I have experienced significant difficulties as a result of it. Throughout my clinical experience, the effective or proper use of health informatics systems got influenced by both the barrier and the enabler.

Role of an enabler
In this context, it must be included in the nursing assignmentthat the enabler, i.e. one of the senior nurses appointed in the organization had helped me gather adequate knowledge of the applicability or implementation use of the Health Information System. Based on my theoretical learning of health information systems, it refers to a strategic structure or system that is usually designed for managing healthcare data (Russ & Saleem, 2018.p.125). I have also learned from thenursing assignmentthat a well-equipped HIS is designed to collect, retrieve store, access, manage and initiate data interaction in accordance with a patient's electronic medical record (EMR) for a hospital's operational management based on a system supporting healthcare policy decisions (Sittig et al.2018.vol 6. pp.10)In a nutshell, the enabler had helped me acquire practical expertise in using Health Information System in a practical work environment.

Role of a barrier
It is also significant to state in the nursing assignmentthat one of my colleagues who was also placed to work as a student nurse in the same organization had initially made me confused about the dynamic use of a Health Information System when I was hardly adequate to deal with patient details and other healthcare data in the beginning.
Reflection on my role as a graduate nurse in ensuring ethical use of health information systems According to the knowledge that I have acquired during my learning session for thenursing assignmentand the clinical experience that I have acquired while I was placed as a student nurse in an organization, I have come to know that it is immensely essential to ensure the ethical use of health information systems or HIS when working as a graduate nurse or registered nurse over the period. Based on my learning while acquiring clinical experience, I have realized through the nursing assignmentit is necessary to protect all significant credentials of the patient or private patient information in order to keep patient safety. My extensive clinical experience has taught me to reflect on the essentiality of initiating effective risk assessment in order to assess potential risk factors while dealing with Health Information Systems strategically (Sheikh et al. 2021.e394). Even, as a graduate nurse, I must accept the liability of assessing the Health Information System, and all the necessary research activities are required to be ethically maintained, especially to ensure accuracy during system implementation.

Part 3: Transition to practice
Analysis of the identified challenges that are often identified

Based on the learning that I have acquired from the nursing assignmentin the form of my clinical experiences, I must admit that working as a professional qualified or registered nurse usually comes with different impediments from both external and internal elements. Even, all of these hindrances are hardly necessarily negative although I must develop professional resilience in order to ensure a fulfilling and sustainable nursing career over the period.

High Patient Ratios
My prior clinical experience encourages me to mention that I had practically noticed HighPatient Ratios when I had been placed as a student nurse in an organization to serve my responsibility. The workplace environment mentioned in the nursing assignmenthas encouraged me to develop my potential and skills (i.e. professional competencies) so that I can effectively handle this kind of clinical emergence at my workplace in the later phases of my career.

Communication Challenges addressed in the nursing assignment
As per my clinical experience and based on the learning while pursuing this course, I have come to know that nurses may experience communication challenges or ineffective communication in the organizational structure in which they have been appointed to serve their responsibilities (White et al. 2021.p.202). In this context, it is significant to state in the nursing assignmentthat the same may cause great difficulties in providing person-centered care to patients effectively. Even, I have also experienced challenges that have been caused due to improper communication in the organizational structure where I got placed as a student nurse.

Identification and critical reflection on two positive workplace behaviors while working as a registered nurse
As per my belief and understanding in the nursing assignmentalong with the clinical experiences that I secured, I think optimistic workplace culture can positively influence the overall effectiveness of entire person-centered care applied to patients.

Helping others as applied to the patients and the colleagues
Based on my learning, I can say from thenursing assignmentthat it is essential to help patients and colleagues in order to positively influence overall operational efficiency and the quality of delivering person-centered care. My prior clinical experience has enabled me to believe that efficient registered nurses tend to fight for patients' rights. Precisely, registered nurses are dedicated to improving and saving the lives of patients.

Proper teamwork environment
In reference to the clinical experiences that I have obtained from thenursing assignmentwhilegot placed as a student nurse and my exhaustive learning on this subject matter has enabled me to believe that a proper teamwork environment tends to positively influence the work efficiency of an organization and the same lead to ensuring effective person-centered care to the patients (Hwang et al. 2019.p.132). In respect to this concern, the same can surely be referred to as a positive workplace behavior that can eventually influence the work efficiency of a registered nurse. Identification and critical reflection on two self-care strategies concerning the maintenance of mental and physical well-being as a registered nurse Based on the critical evaluation and self-reflection drafted in this nursing assignment, it can be claimed that a registered nurse is needed to adopt self-care strategies to ensure proper maintenance of physical and mental well-being.

Patience
Based on my knowledge and understanding, an individual is required to hold on to his or her patience in order to serve the role and responsibilities of a registered nurse (Mariano et al. 2018.p.80). The same will enable the individual to serve efficiently as a registered nurse and will eventually help the individual to ensure his or her mental peace and physical well-being at the same time. Effective communication with others without being judgmental According to my understanding from the nursing assignmentand prior clinical experience, a registered nurse must communicate properly because effective communication can keep mental well-being. Even, it is important not to be judgmental while communicating with others (Lord et al. 2021.p. 102946). This approach may eventually optimize the physical and mental well-being of the registered nurse and surely be considered as one of the self-care strategies while performing the responsibilities assigned to him or her.

Conclusion
Based on the discussion as reflected throughout this nursing assignment, inadequate safety culture tend to cause many vital incidents and it often causes personal injuries as depicted throughout this paper. Even, this nursing assignmenthas reflected on the necessary attributes needed to have a better understanding of Health Information Systems with relevant details. In a nutshell, such exhaustive critical analysis of this nursing assignmenthas been represented in the form of self-reflection based on my clinical experience to date in support of Gibbs' reflective cycle for better understanding.
